Health Card

Your Ontario Health Card should be carried with you at all times as it is key to the provision of insured hospital services.

Pembroke Regional Hospital recognizes that in an emergency, you may not remember to bring your health card. If this is the case, you will have an additional 24 hours to provide it. Regardless of whether you have a health card with you, you will receive treatment.

If you have been a patient here before, and have a Pembroke Regional

Hospital blue card, please provide this.

The Ministry of Health and Long Term Care does not reimburse hospitals for non-emergency procedures when a patient does not have a valid health card. Patients who do not have a valid health card will be billed for their hospital visit.

If you do not have your valid Health Card or have lost it, please contact Service Ontario at 1-888-376-5197 and call the Hospital at (613) 732-3675, extension 6534 with your Health Card number as soon as you receive it.

Hospital Charges

You should also bring appropriate documentation from your Insurance Company to Admitting Department at the time of admission, or within 24 hours.

Physicians bill separately for their services and may request payment at the time of your visit. If this is the case, you will be given a receipt by the physician and may use it to receive reimbursement from the provincial Health Insurance Program.

What are uninsured services and items?

There are certain services and items that require payment upon completion of the procedure or receipt of the items. Prior to receiving uninsured services or items, your service provider will advise you.

Payment of any additional charges can be made at the Finance Office on the 5th floor, Tower A from 8 a.m. to 4 p.m. Monday to Friday, except on Statutory holidays.

Access to quality care will remain equal for all of our patients and will at no time be compromised for those who are unable to provide credit card information. Payment however is required when invoiced.

Admitting/Registration

When you arrive at the hospital, please proceed directly to Admitting, located on the Ground Floor, Tower B beside the Emergency Department. This area of the hospital is open 24 hours a day, seven days a week. The Admitting Clerk will record information such as your name, address, and name of your family doctor and direct you to the nursing unit. Please present your valid Ontario Health Card, blue hospital card (if you have one) and additional insurance coverage information if applicable upon arrival.

You may be admitted directly from the Emergency Department.

Please bring:

Your Health, hospital and insurance cards

Comfortable clothing and non-slip footwear (essential to prevent falls). The Hospital provides hospital gowns, basic hygiene supplies and equipment for patients.

Custom designed aids such as walkers and wheelchairs

bullet

Toothbrush/toothpaste/shampoo/body lotion/shaving supplies, etc.

bullet

Comb and brush

bullet

Facial tissues, sanitary items

bullet

All your current medications and a medication history including drug allergies and sensitivities.

In addition, you may want to bring:

Pen or pencil

bullet

Small amount of money (which will cover approximately one day of patient television service). This fee will be collected by Hospitality Network representatives.

Maternal/ Child Program patients should bring:

Child - favourite toy or blanket, disposable diapers (if required)

bullet

Newborn - appropriate clothing for discharge, including diapers, sleepers and safety-approved car seat

DO NOT BRING:

bullet

Valuables such as jewelry, large sums of money or credit cards. Leave these at home, or with a family member or friend.

Cellular phones

bullet

Alcohol

bullet

Electrical devices/appliances

The Pembroke Regional Hospital is a public building. Although we work hard to maintain a secure environment, Pembroke Regional Hospital is not responsible for lost, stolen or damaged property. If you do bring valuables, they may be temporarily secured in the Hospital safe until you can take them home.

Medication

Patient safety is a priority for our hospital.

When you come to the hospital please bring all your medications in their original containers or a current list including drug allergies and sensitivities. Be sure to include medications you buy without a prescription, all herbal medicines, and health food supplements. Prior to a planned admission to the hospital, please make an appointment with your local retail pharmacist to have a Meds Check completed and bring a copy with you.

The Pharmacy Department at the hospital stocks medications based on an approved list of drugs.

Certain medications may not be readily available and we may ask you to use your own supply while in hospital.


During your hospital stay, the nurses will give you the medication prescribed by the attending doctor at scheduled times. It is important that you not take any other medications, as they may interfere with what you have been prescribed.

Your medications may change while you are in the hospital. If there have been changes, ask for an updated list of the medications you are to take at home and be sure to bring this to your community pharmacist and family physician so they can update your file.

Discharge Planning

Planning for discharge will begin at the time of your admission.

Your healthcare team, in collaboration with you and your family, will determine when you are ready to go home. You will be given specific instructions to follow once you leave the hospital. You may also

be given prescriptions for medication and a follow-up appointment time with your doctor and/or other healthcare professionals. If you brought medications to the hospital, please be sure they are returned to you.

If you are being discharged home, please make arrangements for a family member or friend to pick you up at the regular discharge time of 10:00 a.m. (unless otherwise scheduled) so that other patients requiring admission can be accommodated. If you cannot be picked up at that time, you may be asked to wait in a designated waiting area.

Responsibility of Patient and Family
In order for the discharge process to proceed as smoothly as possible, Pembroke
Regional Hospital would appreciate if you and/or your family would:

Provide necessary clothing to be worn at time of discharge.

bullet

Pack, remove and transport all personal belongings.

bullet

Fill prescriptions for medications prior to, or immediately following discharge. For your convenience, Mulvihill Drug Mart is located in Tower C, the Lorrain Wing. For hours of operation, please call (613) 735-0161, or visit your community pharmacy.

bullet

Be available at the specified discharge time of 10:00 a.m. (unless otherwise scheduled) to take you home.

bullet

Make arrangements for the necessary equipment or home modifications as instructed by the healthcare team.

bullet

Be available to receive necessary instruction for you or your family member’s care prior to discharge.

bullet

Inform the patient care unit of any unforeseen circumstances which may delay discharge.

bullet

In the case of an infant, or small child, have a certified car seat available at time of discharge.
