Once the referral is reviewed and if the client meets the criteria for mental health services, a Choice Appointment is booked with a member of the mental health team.
Clients are encouraged to invite family and care team members to their Choice Appointment, when they will:
At this appointment the client will decide on the the issues they need to address and how best to proceed. This could include group or individual treatment, or a referral to one one of the MHSRC core or specialized services.
Afterwards, the client will receive a letter reviewing what was discussed and outlining their choices. If this includes services with MHSRC, then a Partnership Appointment will be booked to continue treatment. Information regarding other community-based services will also be included.
When treatment goals are met, the client will be discharged.
The CAPA system is designed to provide increased access to services for new and repeat clients. Should a discharged client need additional care, they can be referred for another Choice Appointment.
